pForm.org allows you to create HTML form in fast and easy way. It is a completely free service. Once you enter this site, you’ll see various options of color schemes for your custom form. Choose one to go to the next step, which will be designing your form content.
Click the Untitled Form box to edit the form properties. In this section, you can edit your Form Title and give few lines of form description under this Title tag.
Click one of the form elements button on the right sidebar to add a field. They are:
